Nine Catalan centers are transforming patient care through health outcomes measurement
The Consorci de Salut i Social de Catalunya held a launch session yesterday for the health outcomes measurement project using PROMs and PREMs in patients with immune-mediated conditions. Project leaders emphasized its importance in centering care on the patient and empowering them. This innovative value-based medicine initiative aims to integrate evaluation tools like PROMs (patient-reported health outcomes) and PREMs (patient-reported experiences), while promoting active patient involvement in health decisions and prioritizing health outcomes in clinical choices.
Supported by Sandoz laboratory, the Consorci will implement this in nine facilities: Hospital de Sant Joan de Déu (Martorell), Corporació Sanitària Parc Taulí, Consorci Sanitari Alt Penedés-Garraf, Fundación Hospital d'Olot i Comarcal de la Garrotxa, Serveis de Salut Integrats del Baix Empordà (Hospital de Palamós), Hospital Dos de Maig (Consorci Sanitari Integrat), Hospital Municipal de Badalona (Badalona Serveis Assistencials), Hospital de Mataró (Consorci Sanitari del Maresme), and Fundació Salut Empordà. Hospitals will roll it out in coming weeks, with full operation expected early next year, starting in Dermatology and Rheumatology due to high demand for MHDA medications from hospital pharmacies.
