SEDISA positions PROMs and PREMs as a fundamental basis for more patient-centered healthcare
Sedisa has just presented the report "Positioning on PROMs in the management of Advanced Chronic Kidney Disease (CKD) based on value", which highlights the fundamental role of PROMs (Patient-Reported Outcome Measures) and PREMs (Patient-Reported Experience Measures) in improving healthcare. This report underscores the importance of measuring outcomes and experience directly from the patient's perspective, especially in complex conditions such as advanced CKD, where symptoms and quality of life are decisive factors.
The report emphasizes how PROMs make it possible to capture essential subjective aspects that traditional clinical indicators do not reflect, such as fatigue, pain, pruritus, sleep disorders and emotional state—crucial for patients with advanced CKD. PREMs complement this vision by objectively assessing the quality of care and the patient's experience within the healthcare system. Integrating these measures promotes:
- A person-centered approach focused on quality of life
- Continuous and personalized health monitoring
- Improved communication between patients and healthcare professionals
- Support for clinical decision-making and treatment personalization
- Evaluation of cost-effectiveness and sustainability of interventions
Challenges and proposals for implementation
The report also analyzes structural and cultural barriers such as low awareness, lack of technical training, professional resistance and difficulties in integrating these data into electronic systems. Proposed actions include:
- Homogeneous multidisciplinary training for professionals and patients
- Active patient participation from the design to the use of PROM/PREM
- Development of accessible digital tools integrated into the clinical record
- Establishment of incentives linked to health outcomes
- Promotion of coordinated strategies between autonomous communities and the public–private sectors
Phemium PRM as a strategic ally for PROM and PREM
Addressing these challenges requires mature technological solutions adapted to clinical and operational realities. At Costaisa, we have incorporated PROM and PREM forms into our telemedicine platform, Phemium PRM (Patient Relationship Management), which in just a few years has become the most widely used solution for patient follow-up in Spain.
A kidney transplant is the consequence of a patient with advanced CKD who is in a dialysis stage as a renal replacement therapy. Phemium PRM includes a specific pathway for kidney transplantation that will soon enter its pilot phase.
This platform facilitates the design and deployment of PROM and PREM, the simplified and secure data collection, and their integration with hospital information systems. It also includes tools for analyzing and monitoring prescribed treatments, adapting to the specific needs of each center.
